The Cass Review
The most comprehensive investigation into gender-affirming care for minors ever conducted—and what it concluded.
In April 2024, British pediatrician Dr. Hilary Cass published her final report on gender-affirming care for minors in the United Kingdom. The report was the result of a four-year independent investigation commissioned by NHS England. The conclusions were unprecedentedly harsh: the scientific basis for the dominant treatment protocols is “remarkably weak.”
What the report says exactly
Of the 50 studies on puberty blockers that were reviewed, only one met the minimum quality standards. The report’s conclusion: “No conclusions can be drawn regarding the impact on gender dysphoria, mental and psychosocial health, or cognitive development.” The idea that puberty blockers give young people “time to think” without lasting consequences was not supported by evidence.
Based on the report, NHS England stopped prescribing puberty blockers to minors outside of clinical trials in March 2024. The Tavistock Gender Identity Development Service, the largest gender clinic for young people in the UK, was closed. The BMJ Group concluded that the evidence for puberty blockers and hormone treatment in gender transition is “entirely inadequate.”
Why this receives little attention
In the Netherlands, the Cass Review received hardly any widespread coverage in the mainstream media. The gender clinic at Amsterdam UMC—which laid the groundwork for the globally disseminated “Dutch Protocol”—continued to defend its practices. Critics point to the institutional interests at play: clinics, scientists, and activist organizations that have invested in the existing model for decades.
The Cass Review is not a political pamphlet but a medical-scientific report commissioned by the national health service. Ignoring or downplaying it in the public debate is an example of how ideological pressure can supplant scientific conclusions.
